New Patient Information
What To Expect on Your First Visit:
You should arrive 15 minutes early to fill out paperwork. We suggest wearing comfortable clothes. Your first visit will be your evaluation. It will take about 1 hour or longer, depending on your condition or injury and if therapy follows.

Requirements on Your First Visit:
You will need insurance cards and photo ID. A minor will need to be accompanied by an adult. We do require that all co-pays and/or deductibles be paid at time of service.
